GERIATRIC SERVICES

After completing all forms and sending them to Adonai Psychological Services, contact APS at (281) 651-1700 to make a day or evening appointment.

IF any adult client is under the conservatorship, provide documentation of the conservatorship and complete the consent for geriatric treatment.

You can fill it out electronically, save it on your computer, email it as an attachment it to newpatient@adonaips.com, and then sign it when you come to the office.
OR
You can fill it out electronically, print it out and sign it, and bring it to the office with you.
OR
You can print it out, fill it out and sign it with a pen, and then bring it to the office with you.
