CHILD / ADOLESCENT SERVICES

Dr. Schloneger provides psychological testing and therapy for 5-18 year olds.

One of the following is required from the caretakers of minors:

Birth Certificate
Divorce Decree Stating Parental Rights
Guardianship or Adoption Documentation

Foster Children: the Child Protective Services caseworker must complete all required forms. The foster parents or appropriate caretakers are expected to participate in all psychological services.

Most recent report card
Prior report cards (helpful but optional)
Medications and their dosages (current & prior)
School discipline reports & A.R.D. documentation
For Special Education:school psychological evaluations or diagnostician's reports
Bring all medical, psychological, school, etc. documentation that will help with the understanding of your child

You can fill it out electronically, save it on your computer, email it as an attachment it to newpatient@adonaips.com, and then sign it when you come to the office.

You can fill it out electronically, print it out and sign it, and bring it to the office with you.
